If you have a severe mental health problem (applying under category 5) please send your completed application form to the address below. After this, you will be contacted to arrange a date for an assessment.

If you are assessed as being eligible for a Freedom Pass a letter of authorisation will be sent to you by post. This letter will need to be taken to any post office in the London Borough of Barnet to obtain your pass.
Please note that if you are not known to Barnet Adult Social Services, your GP may be contacted for information regarding your condition.
