The support plan identifies how you would like to use the money given to you by Adult Social Services through your personal budget. It describes the support you think that you need to meet your social care needs. It will say who will provide this support, how much it will cost and how it will be arranged and managed.
When you write a support plan think about the needs you identified within your personal budget questionnaire and what goals you want to achieve.

Once you have completed the support plan, you need to show it to your social worker and care manager. They must agree with the needs you have identified and how you propose to support them and sign your support plan.
When the Support Plan is agreed your Personal Budget amount will be finalised and you will be informed of what are the next steps and when the Support Plan will be reviewed. If the Support Plan is not agreed you will be informed of the reasons why and what action can be taken.

If you feel that your support plan is not meeting your needs and changes need to be made, you should review your support plan with your social worker. If at any point your needs increase or decrease please talk to your social worker or care manager and you may need to complete another personal budget questionnaire.
