Carer's assessment
As a carer you are legally entitled to a carer’s assessment. The carer’s assessment looks at your needs to find out if there are any areas in your life that you would like help with. If any of your needs are identified in your assessment then Adult Social Care and Health may be able to help in many ways. For example, we could provide you with:
- help in the home
- a break from caring
- information and advice
- equipment and financial assistance.
We can also put you in contact with specialist agencies that can offer you services such as outings, holidays, training, individual counselling, social events and support. We may also be able to provide services to the person you care for to make things easier for you.
If you are a young carer (under 18 years) we can put you in contact with the Barnet Young Carer’s Project where you can get help with any area of your life and meet other young carers if you wish to. To work out how you can be helped and supported in your situation, we to discuss:
- the needs of the person you care for
- the help and support you are giving
- what you feel will make a difference to you.
What your assessment is for
A carer’s assessment is your opportunity to tell us about the things that could make life easier for you. The Carer’s Assessment Form (below) can be completed by you in private and sent back to us. If you prefer a social worker or health care professional can complete it with you. Alternatively, you can contact one of the support organisations for carers to assist you with the form.
The assessment will outline a plan of action at the end of the form which will clearly state how you can be helped and what contribution we, at Adult Social Care and Health, can make. You should receive a written copy of your assessment at a later date.

Completed forms then need to be given to the health or social care worker who gave it to you, or send it to the relevant social work team below.
