A medical certificate of stillbirth issued by the doctor or midwife.
Who can register a stillbirth
If the parents are married:
- the mother
- the father
If the parents aren't married:
- the mother
- the father if a 'signed declaration' has been made by the mother

What is the difference between a stillbirth and a death?
A baby is stillborn when it is born after the 24th week of pregnancy and does not breathe or show any other signs of life after birth. A death is when a baby has lived, no matter how long, and subsequently dies. Therefore if a child is born alive and only lives for a matter of minutes the event must be registered as a live birth and a death.
