Children and young people in Barnet with physical and/or medical needs.
How is a child referred to the team?
Pre-school aged children are referred to the team through any professional via the multi agency intake meeting at Finchley Memorial Hospital
School aged children are referred through to the team via the school using the “Request for Involvement” form
How can the team support a pre-school child?
- Home visits by the pre school advisory teacher
- Joint visits with parents to different early years settings
- Arranging attendance at parent groups
- The pre school teacher is the key worker for some children with physical disabilities
How can the team support a pre-school setting?
- Advise staff on physical disability
- Attend Individual Education Plan (IEP) meetings
- Attend a child's Annual Review
- Staff training
- Joint working with other professionals
- Support the transition to nursery or school
- Advise on health care plans
- Advise on resources
How can the team support a pupil at school?
- Help the pupil access the curriculum
- Help the pupil with access to the buildings
- Work with other professionals
- Support the pupil with secondary transfer or transition to college
- Liaise with schools offering outreach
