My Health Matters in Barnet

Zoom

In Barnet, our aim is to improve health outcomes for people with a learning disability. We know from the Confidential Inquiry (CIPOLD) that people with a learning disability are dying prematurely. This should not be happening.

There are many reasons for this, including:
• lack of healthcare co-ordination
• communication barriers
• lack of reasonable adjustments being made by mainstream healthcare services

Zoom

It is recommended that all people with a learning disability should have an annual health check with their GP and a health action plan.

A Health Action Plan is a document that contains important information about someone's health needs. The  action plan should be taken to all  health appointments. This will make sure vital information is shared and help health care staff understand how to work best with that person.

The My Health Matters book is Barnet’s approach to health action planning and was launched in 2018. Find out more about annual health checks
