Conditions which prevent you from obtaining a driving licence
If you do not fully complete the application form or if you fail to provide the supporting evidence your eligibility cannot be determined and your application will be closed after a period of 4 weeks from the date your application is received.

Please provide medical evidence which relates to the conditions you have selected below. This evidence should be current and from a medical professional who is able to confirm your diagnosis, prognosis, treatment plan and how this disability prevents you from being able to drive.
