Carers assessment

A carer's assessment is our way of finding out if you can get support in your role as an unpaid carer.

It is an opportunity to talk about how caring responsibilities affect you. The carer's assessment will look at the following:

• If your caring responsibilities have an impact on your wellbeing

• If you are willing and able to continue providing care

• Any outcomes you wish to achieve in your day-to-day life

• If you need any support and what you can get from us.
