Your offer is subject to completing a number of pre-employment checks. You will find below a number of forms that must be read and/or completed by you in order for us to comply with our statutory obligations and to enable us to set you up on our payroll system.

You must complete all the relevant forms and instructions in the list below and email to arrange a suitable time to bring in your completed forms together with the original documents that are requested below.

Online Conditional Offer and Contract, and Online Applicant OnBoarding Form
You should have already read and accepted your on-line conditional offer & contract and completed the online Onboarding Form. If not then please do this first. The instructions on how to access these on-line forms were given to you in a separate email

Accept your on-line Conditional Offer and Contract and complete the online Onboarding Form (provided in a separate email)


Online Pre-Placement Medical questionnaire:
Please follow the instructions in your conditional offer letter on how to access this questionnaire. 

Complete the online questionaire (link in your conditional offer letter)


Online Disclosure and Barring Service (DBS) check:
This check is only required for certain posts. If you are required to complete this check this will be outlined in your conditional offer letter along with the instructions on how to do this. You will also be required to provide the original identification documents as outlined in the DBS List of Acceptable Identification


If relevant to your post, complete the online DBS check and provide the original Identification documents


Evidence of a permanent National Insurance Number
You are required to provide evidence of your national insurance number. We are not able to accept a National Insurance CardSee our list of acceptable documents.

Evidence of your Right to Work in the United Kingdom
You are required to provide evidence of your right to work in the UK. See our list of acceptable documents

Photographic ID
This is only required if your Right to Work documentation does not include a photograph

Evidence of a relevant Qualification

If relevant provide the original qualification certificate

Local Government Pension Scheme (LGPS)
To join the LGPS please complete the pension form(PEN1) and the Expression of Wish Death Grant form(PEN16).

Under the Government pensions auto-enrolment legislation, all employees who meet specific criteria of age and earnings, will be automatically enrolled into the appropriate pension scheme.

If you do not wish to join the LGPS please complete the Opt Out form (PEN4) and email it to Pease note, you cannot complete the Opt Out form until after you have started work.



P45 OR HMRC Starter Checklist Form
To ensure that we put you on the correct tax code you must submit a P45. This is usually obtainable from your current/most recent employer. If you do not have a P45 you will need to complete the HMRC New Starter Checklist form (PDF)
If you do not submit a P45 or the HMRC New Starter Checklist form you may pay too much tax as it will be deducted at the basic rate.

Please email to arrange a suitable time to bring in your completed forms and the original documents that have been requested. Your documents will be verified by the HR team who will take a copy of your documents and return the originals to you
