Our Service

The Transition Hub offers a tailored educational programme for adolescents (Looked After Children or LAC) new to care or who are in care but experience a school or carer placement change, between the ages of 11 and 15. The staff at the Hub, run at Barnet in Whitefield School, provide a phased transition for students to a new school, initially, over a period of 6 weeks and then with outreach support for up to 6 months. For adolescents who are settled in school, an outreach programme is offered to support the transition into care or a new carer placement for up to 6 months.

The project is designed to benefit both children and society through improving life outcomes for children in the programme and reducing the likelihood of youth offending.

The Transition Hub is a new approach being trialled with government funding, through the Youth Endowment Fund. There is an evaluation study which is a required element and will assess whether the Hub provides the expected benefits; the provision of effective; and efficient support.

The council is the data controller for the data processed to run the Transition Hub and provide support to the children.

The University of Plymouth is data controller for the evaluation project and the council shares data with the University so they can carry out the evaluation.

Contractors

Our contractor Whitefield School works on behalf of the council to host the Transition Hub within their secondary school building.

How long we keep your information

Information will be stored on the child’s Family Services and Virtual School case files. The LAC retentions for casefiles are 75 years once the child turns 18 years old.
