FOI - Household Income Assessment

I would like to have a copy of the policy (and/or guideline and/or training material, or any titled document that substantially covers the same topic) provided to nurseries in the borough on calculation methodology of annual household income for income charge assessments and childcare charges purposes current as of the request date. In other words: how are parents'/carers' incomes assessed to determine the household income for banding purposes; what is/are the methodology/ies and/or information used to calculate the annual income of the household?

In case of agency workers and workers contracted on zero hours usually receive their holiday entitlement in payments (not as paid annual leave), and usually these are on the payslips used for assessing their income. Whatever calculation may be used by the employer, is the holiday entitlement paid to such worker counted towards their annual income for determining their annual income for income charge assessments and childcare charges purposes?
